Super Speech HQ provides speech pathology services to children and their families.
Identifying and contact details (name, address, phone, email).
NDIS number and funding information.
Case history, medical and developmental information.
Session notes, assessment results, reports.
Audio, photo, and video recordings (only with your consent) for assessment, clinical planning, and progress tracking.
To provide safe, effective therapy and coordinate care.
To communicate with you about appointments, invoices, and progress.
To meet reporting obligations to you, the NDIS, Medicare, or other agencies where relevant.
We rely on your consent to collect and use this information, unless required or authorised by law. You may withdraw consent at any time, though this may affect our ability to provide services.
Directly from you or your authorised representative.
From other providers or professionals with your consent.
If we receive information from others, we will take reasonable steps to let you know.
We use your information only to deliver and coordinate care.
Acceptance of this policy indicates your consent for us to disclose information to teachers, childcare educators, GPs, paediatricians, psychologists, allied health providers, and NDIS personnel, unless you have otherwise requested that we do not in writing.
We may disclose information without consent if there is serious risk to a person, suspected abuse or neglect, or where otherwise required by law.
Records are stored in password-protected digital folders or password-protected client management software.
We may use secure third-party services (such as Google Workspace or Splose) to store information. These services may store data on servers in Australia or overseas. We take reasonable steps to ensure your information remains protected.
Access is limited to the treating clinician.
We retain adult records for 7 years after the last service, and children’s records until age 25, in line with professional and legal requirements.
Email is convenient but not perfectly secure. You may opt out of email communication at any time.
If you remain opted in, you authorise us to send reports, invoices, and other information to the addresses you provide.
We can password-protect documents on request.
You may request access to your records at any time.
We will respond within a reasonable period and may charge a reasonable cost-recovery fee for copying.
If information is inaccurate or incomplete, you may request correction.
If a data breach occurs that is likely to cause serious harm, we will notify you and the Office of the Australian Information Commissioner (OAIC) in line with the Notifiable Data Breaches scheme.
